Applying for Design Professional Membership is easy:

1. Type in the form fields right on the screen of the Application Form found here . Use the TAB key to jump between form fields.
2. Select your method of payment, filling in the required credit card info if paying by card.

3. Print the completed form and sign it.
4. Mail / fax the printed form to AF&PA/AWC using the contact information on the bottom of the form.
5. Don't forget to include your check for the membership fee, if you're paying by check. Please do not send cash.

3. SAVE the completed form on your computer.
4. Email the saved form as an attachment to: AF&PA Member Relations
